Art thérapie en unité de soins palliatifs : un accompagnement possible jusqu'au stade préagonique

RésuméDepuis plus de deux ans, l'unité de soins palliatifs de Cébazat propose de l'art thérapie aux personnes hospitalisées dans le service. Les séances sont assurées par une art thérapeute qui, en fonction de l'état général du patient, de son autonomie, ses capacités cognitives, et sa demande, développe un dispositif unique, permettant à chaque patient de s'exprimer par l'art. Méthode essentiellement non verbale, cette approche permet à la personne gravement malade, dont la souffrance physique et psychique sont bien souvent innommables, de se dire à travers les couleurs, les matières, les sens… En unité de soins palliatifs, cette approche soulève un grand nombre de questions et cet article interroge la légitimité d'accompagner un patient par l'art thérapie jusqu'au stade préagonique. Une étude de deux cas cliniques complets témoigne de la portée et la nécessité de ne pas cesser l'accompagnement lorsque le patient entre dans ce stade. Elle montre combien à l'aube de sa mort, communiquer reste fondamentale, et comment l'art thérapie l'y invite.

SummaryArt therapy has been practised in the Clermont-Ferrand hospital palliative care unit for two years. Art therapy workshops are offered by an art therapist. Her evaluation of the general status, autonomy and cognitive abilities of the patients, but also what they really want are all the conditions required for the art therapist to develop a special way to allow patients to express themselves through an artistic production. Of course, the aim is not the production of art itself, but this is rather a non-verbal method of communication through colours, substances or feelings for patients with a serious disease, who suffer both physically and mentally in an unbearable way. Nonetheless, in the palliative care unit, this kind of care approach may be questioned. This article is intended to examine the legitimacy of practicing art therapy until the preagonal phase. Two fully-reported cases are studied to demonstrate how this can be useful and with what results, even if the patient is in a preagonal situation. Communication is essential and must be preserved until the end of life.
