Rationalisation des voies d'administration du paracétamol : quelles actions ? Quel impact à l'hôpital ?

RésuméIntroductionUn audit de pertinence d'utilisation de la voie injectable du paracétamol réalisé en mai 2012 a montré que la prescription de la forme injectable n'était pas pertinente dans 36 % des cas. L'objectif de ce travail est de recenser l'ensemble des actions menées pour rationaliser l'usage des voies d'administration du paracétamol et d'en évaluer l'impact sur les consommations de paracétamol injectable et oral.Matériels et méthodesParallèlement, au recensement des actions de rationalisation des modalités d'administration du paracétamol, les consommations des différentes formes galéniques ont été analysées rétrospectivement entre le 1er janvier 2007 et le 31 décembre 2014. Au vue des données, une étude de minimisation des coûts a été réalisée.RésultatsNeuf actions multiservices ont été relevées. Si la consommation totale de paracétamol a augmenté de 53,7 % en huit ans, le ratio de la forme injectable sur la forme totale a significativement diminué, passant de 24,3 % en 2007 à 11,5 % en 2014. Le montant des coûts évités sur cette période s'élève à 678 297 €.ConclusionCes actions contribuent à améliorer le bon usage du paracétamol tout en diminuant les coûts associés.

SummaryA relevance audit of paracetamol injectable way realized in May 2012 showed that the prescription of the injectable shape was not relevant in 36 % of cases. The objective of this work is to list all the actions carried out to rationalize the ways in which paracetamol is used and to estimate the impact on the consumptions of injectable and oral paracetamol. In a parallel to the inventory of these actions to rationalize the modalities of paracetamol administration, the consumptions of the various pharmaceutical forms were analysed afterward between 1st January 2007 and the 31 December 2014. In view of the data, a study of costs' minimization was realized. Nine multi-departments actions were counted. If total, paracetamol consumption increased by 53.7 % in 8 years, the ratio of the injectable form on the total form significantly decreased, passing from 24.3 % in 2007 to 11.5 % in 2014. The amount of the costs avoided over this period amounts to 678,297 €. These actions contribute to improve the good use of paracetamol while decreasing the associated costs.
