RésuméL'émergence de nouvelles molécules a considérablement remodelé la prise en charge des patients en onco-hématologie. Les chimiothérapies cytotoxiques n'ont pas été modifiées, et le CHOP reste le traitement de référence des lymphomes. Cependant, le développement de thérapies ciblées a permis un élargissement du spectre des traitements. L'immunothérapie avec les anticorps monoclonaux ont fait leur entrée avec le rituximab, dans les lymphomes B diffus à grandes cellules, dans les années 1990 et sont en développement à l'image des anticorps anti-CD20 de nouvelle génération (obinotuzumab et ofatumumab). Les anticorps anti-CD30 ont été proposés dans le traitement des lymphomes T et des lymphomes de Hodgkin. Plus récemment les anticorps anti-PD1 ont apporté de nouvelles perspectives dans plusieurs cancers et plus précisément dans le lymphome de Hodgkin. Enfin l'inhibiteur de BTK, l'ibrutinib développé dans la LLC s'est imposé dans la prise en charge du lymphome à cellules du manteau et de la maladie de Waldenström. Comment s'y retrouver avec toutes ces nouvelles molécules ? Faut-il les proposer en monothérapie, en association ? En première ligne ou en rechute ? L'objectif de cette revue est de permettre de retracer les dernières avancées, et de mettre en lumière les stratégies validées représentant les nouveaux standards de traitement des lymphomes en 2017.

Emergence of new molecules has considerably reshaped the management of patients in onco-hematology. Cytotoxic chemotherapy has not been altered, and CHOP remains the reference treatment for lymphomas. However, the development of targeted therapies has allowed for a broader spectrum of treatments. Immunotherapy with monoclonal antibodies entered the market with rituximab in diffuse large B-cell lymphomas, in the 1990s and it is now developing as new-generation anti-CD20 antibodies (obinotuzumab and ofatumumab). Anti-CD30 antibodies have been proposed in the treatment of T lymphomas and Hodgkin lymphomas. More recently, anti-PD1 antibodies have brought new perspectives in several cancers and more specifically in Hodgkin's lymphoma. Finally the BTK inhibitor, ibrutinib developed in the LLC has established itself in the management of mantle cell lymphoma and Waldenström macroglobulinemia. How can we deal with all these new molecules? Should they be offered as monotherapy or in association? In first line or relapse? The objective of this review is to trace history of the latest advances, and to highlight the validated strategies representing the new standards of treatment of lymphomas in 2017.
