The following definition, results, proofs, and example have been corrected or revised: proof of Lemma 2.7, statement and proof of Theorem 2.8, the definition of irreducible ideal, statement and proof of Theorem 2.15, Example 2.17, Theorem 3.8, statement and proof of Corollary 3.9, statement of Theorem 3.15, and statement and proof of Corollary 3.16. The following items have been added: Definition A, Lemma B, and Example C.
