Abstract
Reason: Parts of the discussion section are identical or nearly identical to the contents of a paper that had previously appeared online in the Web site Medscape at http://www.medscape.com/viewarticle/458630. The authors, Dr Amor Khachemoune, who was responsible for the preparation of the discussion section, and Dr Carlos Rodriguez, whose role was limited to the preparation of the questions, regret this inadvertent oversight. We apologize to readers of the journal that this was not detected during the submission process.
