Le médecin, le patient et son proche. Enjeux conjugaux et arrangements médicaux

RésuméL’article s’attache à analyser le déroulement d’une consultation d’un patient ayant une maladie de Parkinson associée à des troubles psychiques et cognitifs. Par le biais de leurs diagnostics, le patient et son épouse s’accusent réciproquement d’être à l’origine de la crise conjugale qu’ils rencontrent et tentent d’user de l’autorité médicale pour infléchir les rapports de force qui les opposent. Ces affrontements portent sur l’autonomie décisionnelle du patient. Le neurologue s’appuie sur un jugement médical et moral pour décider qui, du patient ou de l’épouse, est le plus à même de définir la situation. Les prises de parole des époux Baron en consultation sont alors mises en regard avec leur situation au domicile, situation de domination sociale de M. Baron sur son épouse prise elle-même dans des normes de solidarité conjugale. L’analyse du cas des Baron révèle les conditions qui permettent au patient d’imposer son point de vue en consultation et au domicile.

The article deals with the observation of a patient's consultation, diagnosed of Parkinson's disease associated with emotional and cognitive disorders. Through their profane diagnosis, the patient and his wife accuse each other of being responsible for the conjugal crisis they face. They try to use medical authority to influence the balance of power. Clashes deal with patient's ability to decide. The doctor decides who is the most able to define the situation: the patient or the spouse? He bases his decision on medical and moral judgments. Mr and Mrs Baron's interventions during the consultation and the impact of interactions between doctor, patient and close relative are viewed in the broader context of caring household. At home, Mr Baron has a social dominant place and his wife respects norms telling about conjugal solidarity. The case-study of the Baron leads us to understand which conditions are necessary for the patient to be listened by the doctor and his wife, to safeguard his ability to decide, in consultation and at home.
