► The cost-effectiveness of extended pertussis booster programs was estimated. ► An age structured transmission dynamic model was used. ► A booster dose at 12 years can be considered as cost effective (€4200/QALY). ► Other considered booster strategies could also be labeled as cost-effective. ► The impact of assumptions regarding disease related parameters was limited.
