Abstract
This work explores the subjective effects of neurologically determined hallucinatory manifestations, in a patient with Lewy body dementia, Ms M., encountered in outpatient psychotherapy in a hospital Neurology department. While in the process of Alzheimer's disease delusional and hallucinatory manifestations often appear at a late stage and could be a defensive and compensatory logic in the face of the neuro-psychic disintegration, hallucinations often occur very early in dementia with Lewy bodies, even before the cognitive functions are disturbed, on account of their neuro-organically determined character. However, this organic causation does not exclude the implication, on another level, of psychic causality. Thus, the meaning of these hallucinations could be underpinned by projective and dreamlike mechanisms, as in the case of Ms. M, providing some guidance in the psychotherapeutic approach of these patients.
