Since the announcement of governmental notifications for recognizing and preventing circulatory diseases from overwork, many Japanese companies started to realize the health impact of long overtime work and to consider its effect on risk factors of circulatory system diseases. In this study, we reviewed relevant articles and statistics and developed a practical questionnaire for general health surveillance at real workplace to assess the effect of overtime work. Based on the findings of literature, we included the questionnaire asking sleeping hours, dietary habits, physical activities, general health questionnaire (GHQ), and the Breslow's 7 health practices as practical measures. These information collected by the questionnaire should be stored in a medically confidential health file.
