Abstract
Fluid management can determine the evolution of ALI/ARDS patients. Fluid volume corresponds to the main risk of pulmonary worsening in case of increased pulmonary permeability. Daily fluid balance must be adequately assessed in prevention of excessive fluid overload. The best fluid to be used remains undetermined for the preservation of pulmonary function. Transfusion indication must be clearly assessed avoiding fluid overload and acute lung injury occurrence. Renal function preservation appears essential for the prevention of excessive positive fluid balance promoting poor outcomes.
