Abstract
During the 10 last years, incidence of very and extremely preterm newborns increased from 1.2% (1.0-1.4) in 1995 to 2,0 (1.8-2.2) in 2003. In region des Pays de la Loire, west region of France, prematurity after medical decision increased significantly from 40 to 50% during this same period of time. Advances in prenatal care have improved survival rates each year (adjusted ORÂ =Â 1.1 [1.01-1.21], PÂ =Â 0.02). In 2005, at Nantes university hospital, neonatal mortality was 44% for preterm less than 27Â SA of gestation (24-26Â SA), 14% for 27-28Â SA, 2% for 29-30Â SA and 31-32Â SA. At two years, 8% were lost in follow-up, 80% had a normal examination and 12% an abnormal or suspect examination. The man goal for the coming years will be to improve the quality of neonatal and post discharge care in order to improve the long-term outcomes of very preterm infants.
