RésuméContexteLa lombalgie est un des motifs principaux de consultation chez le kinésithérapeute et constitue un véritable problème de santé publique. Il n’existe pas actuellement en France de recommandations sur la prise en charge des patients lombalgiques qui permettent de rassembler les différentes présentations cliniques dans des sous-groupes homogènes. Une telle classification permettrait que le bilan habituellement réalisé par les kinésithérapeutes devienne réellement diagnostique, pour proposer des traitements adaptés et efficients.ObjectifNous proposons d’utiliser la classification de l’APTA (American Physical therapy Association) parue en 2012 pour guider l’évaluation et le traitement des patients lombalgiques. Cette classification présente une organisation en tableaux cliniques, apportant un intérêt thérapeutique mais aussi didactique pour la construction des enseignements sur la lombalgie. Nous présenterons la démarche de classification selon les différents tableaux cliniques, les moyens diagnostiques et les principes de traitements adaptés.Niveau de preuveNon adapté.
SummaryBackgroundLow back pain is one of the main reasons for consulting a physiotherapist and constitutes a real public health issue. In France, there are currently no guidelines for the management of patients suffering from low back pain that enable varying clinical presentations to be classified in homogenous subgroups. Such a classification could allow the clinical assessment usually performed by physiotherapists to become genuinely diagnostic, enabling appropriate and effective treatment.ObjectiveWe suggest implementing the APTA (American Physical Therapy Association) classification system, published in 2012, to guide the evaluation and treatment of patients suffering from low back pain. This classification, organised according to clinical presentation, is not only of therapeutic but also of educational interest for structuring training courses in low back pain.
