Abstract
Type 2 diabetes mellitus is a chronic disease that often predisposes the patient to more serious cardiovascular conditions, such as myocardial infarction and stroke. Continuity of care has been proposed as a way to reduce hemoglobin A1c levels in the type 2 diabetes patient. This article reviews and summarizes the literature on how continuity of care affects hemoglobin A1c levels in patients with type 2 diabetes mellitus. Practice recommendations will be provided to support nurse practitioners in shaping their local practice policies.
