Abstract
The Triple Aim is a framework for optimizing a health system's performance by “improving the patient care experience, improving the health of populations, and reducing per capita cost of health care.”1 Within this framework, health systems strive to provide integrated, coordinated, and client-focused care while bridging acute and community care.
