Abstract
Hospital readmissions are common and costly. Sometimes the readmission is appropriate, but many are avoidable. Approximately 1 in every 5 discharged Medicare patients are readmitted within 30 days, and 1 in 3 adult patients discharged from the hospital to home do not see a physician within 30 days of discharge.1, 2 Some of these patients are well known to your staff as frequent flyers, but why do they come back? Why are these patients high utilizers of the healthcare system?
