Abstract
Early event reporting and investigation of adverse events are essential to an effective patient safety program. Nurses have a role to play when patient harm has occurred in initiating and participating with investigations. This article reviews six steps of the investigation process, tips, and roles nurses can play that lead to successful patient harm investigations and subsequent implementation of lessons learned.
