ZusammenfassungIn der Diagnostik einer Stenose der A. carotis interna ist die Duplexsonographie als fester Bestandteil in der Routinediagnostik etabliert. Dies gilt insbesondere auch in der Nachsorge von Patienten, bei denen ein revaskularisierender Eingriff (operative Thrombendarteriektomie oder stentgeschützte Angioplastie) durchgeführt worden ist. In der vorliegenden Übersicht wird dargestellt, auf welche möglichen Fallstricke hierbei geachtet werden sollte. Sowohl nach einer Operation der A. carotis interna, als auch nach einer Stent-Intervention, ergeben sich eingriffspezifische Besonderheiten, deren Kenntnis für eine duplexsonographische Nachuntersuchung wichtig ist. Dies schließt sowohl die interventionsspezifischen Aspekte der jeweiligen Methode, als auch z.B. individualisierte Re-Stenosekriterien der Duplexsonographie mit ein. Insbesondere innerhalb des ersten Jahres nach einem Eingriff sollte hier eine engmaschige duplexsonographische Kontrolle des Patienten erfolgen.
SummaryDuplex sonography is considered as a routine diagnostic tool in the diagnostic work-up of carotid artery stenosis. This method is also especially valuable and broadly established in the long-term follow-up care of patients following revascularising procedures such as surgical carotid thrombendarterectomy or interventional carotid artery stenting. Therefore, the present article serves to delineate potential pitfalls during follow-up vessel examination in these patients. Specific features associated with either surgical or interventional carotid artery revascularization procedures are of importance to the investigator and have to be considered during carotid duplex sonography. This incorporates both specific technical aspects of either of the procedures as well as for example individualized criteria in the detection of restenosis with duplex sonography. Especially during the first year after an intervention or surgery, close follow-up visits including duplex sonography are recommended for all patients.
