Abstract
Beyond the rehabilitation phase, the goal is to achieve real social integration. In our opinion, patient-centered rehabilitation care should evolve into the development of an integral life project. Evaluation tools are constantly being developed to describe the neurological, motor and neuropsychological disorders related to stroke. The system for identifying and measuring handicap (SIMH) that we propose enables an assessment of the lesions and functional problems involved as well as the subjective situation experienced by the stroke victim. SIMH is adapted not only for rehabilitation purposes, but also as a tool for a better integration into familial, social and educational and occupational life.
