Abstract
An individualized and flexible entity, the process of projecting a patient's discharge is an integral part of medical care. The elements underlying this construct and their implementation belong to the domain of social representations. The patient's discharge involves an articulation between, on one hand, the health care facility and the medicosocial context, and on the other, the patient's own resources expressed in terms of social participation. The process of projecting discharge, with successive verifications of its chances for success, is in itself a personalized factor of the rehabilitation process.
