Abstract
												Inadequate surgical margins in breast-conserving surgery for breast cancer have been identified as a risk factor of local recurrence. There is still no consensus on what constitutes an adequate surgical margin, however, there is a trade-off between widely clear margins and acceptable cosmesis. Reexcision can lead to increased psychological trauma for the patient, delay of adjuvant therapy, worsened cosmesis, and increased costs. Imaging has a prominent role in treatment strategies. Can imaging contribute to decrease the number of reexcision for positive margins? We will approach alternately the role of the preoperative imaging assessment, preoperative location and the intraoperative specimen assessment in the optimization of surgery. Finally, we will describe the potential role of the post-excisional imaging to identify the residual disease.
