Allergie aux hyménoptères : quel diagnostic?

RésuméL'allergie aux piqfûres d'hyménoptères regroupe l'ensemble des manifestations cliniques en rapport avec une sensibilisation à certaines protéines de leur venin.Des recommandations (aspects diagnostiques et approche thérapeutique) ont été publiées sous la direction d'un groupe de travail de l'Académie Européenne d'Allergologie. Après un rappel des manifestations cliniques et des méthodes diagnostiques à notre disposition nous proposons une discussion sur le thème du circuit patient. En effet seul un nombre limité de patient bénéficient d'explorations spécifiques permettant une prise en charge complète et sécurisée. Le problème de l'après urgence dans les cas graves et celui des patients è gravité intermédiaire régulièrement sous-évalués seront évoqués. L'approche d'une prise en charge complète proposée par le réseau régional ALLERG.O.S sera discutée afin d'optimiser l'accès au soin dans un objectif de diagnostic et d'évaluation du risque de réaction fatale.

Allergy to hymenoptera stings brings together all the clinical manifestations associated with sensitization to certain proteins in the insects'venom. Recommendations dealing with diagnostic aspects and the therapeutic approach have been developed under the direction of a working group of the European Academy of Allergy and Clinical Immunology. After reviewing the known clinical manifestations and available diagnostic methods, I will then discuss the subject from the patient's point of view. In fact, only a limited number of patients benefit from a definitive workup that results in complete and safe management of the problem. The problem of post-emergency treatment for serious cases and for cases of moderate severity that are regularly underestimated will be discussed. The approach to all-inclusive management proposed by the ALLERG.O.S regional network will be discussed so as to optimize access to care with the aim of diagnosis and evaluation of the risk of a fatal reaction.
