RésuméLe test de provocation par voie orale (TPO) aux aliments représente le test de référence pour le diagnostic d'une allergie alimentaire. Il permet aussi d'évaluer la tolérance à un aliment chez un enfant sensibilisé ou antérieurement allergique. Il comporte des risques pour le patient. À partir des données de la littérature et pour les principaux aliments impliqués dans l'allergie alimentaire chez l'enfant (lait de vache, œuf de poule, arachide), ce chapitre a pour objectif de préciser les indications, selon la situation clinique et les résultats du bilan allergologique (IgE spécifiques, prick-test, atopy patch tests), et les contre-indications du TPO.

Oral food challenge (OFC) is the gold standard to diagnose food allergy. The OFC defines also the tolerance to a particular food in a child previously sensitized or allergic. There are significant risks associated with OFC. According to the literature review and for the three major foods implicated in food allergy in children (cow's milk, hen's egg and peanut), this chapter analyzes the indications, taking into account the clinical situation and the results of allergy testing (specific IgE assay, skin prick tests, atopy patch tests), and the contra-indications of OFC.
