Abstract
We report 3 patients with POEMS syndrome that illustrate the multisystemic, chronic and progressive nature of the disease. Time since onset was between 2 and 4 years. The patients complained of fatigue, lethargy, weight loss, followed by progressive weakness and skin changes. At diagnosis, all 3 patients had multiple endocrine compromise (hypothyroidism, adrenocortical insufficiency and hypogonadism).The literature highlights the frequency of endocrine abnormalities, which are present in two-thirds of patients at presentation. The most common feature is hypogonadism followed by adrenocortical insufficiency and hypothyroidism. Recognition of the components of the syndrome (neuropathy, organomegaly, endocrinopathy, M protein and skin changes) is the first step in managing the disease. Treating the endocrine compromise is essential.
