The aim of this paper is to justify the adoption of the carotid endarterectomy as the first line treatment for carotid bifurcation disease in the endovascular era. This is based on a non-systematic review of the literature, the analisis of the main randomized controlled trials published, and the guidelines of the most relevant scientific committees.
