Diverticule congénital isolé du ventricule gauche découvert à l’âge adulte : apport de l’imagerie

RésuméLe diverticule cardiaque est une malformation congénitale rare, sa découverte à l’âge adulte est encore plus rare. Il est souvent associé à d’autres diverticules thoraco-abdominaux, et rarement isolé. Le diagnostic repose sur l’échocardiographie. Par ailleurs, l’imagerie par résonance magnétique (IRM) permet une analyse plus fine du diverticule, de sa topographie et ses rapports avec les structures vasculaires et cardiaques, elle a largement remplacé la ventriculographie. Nous rapportons dans cet article le cas d’un patient de 36 ans dont le diagnostic de diverticule a été posé par l’échocardiographie transthoracique (ETT) lors d’un bilan de dyspnée, cet examen a aussi objectivé une insuffisance mitrale (IM) par déformation et dilatation de l’anneau mitral. Le bilan radiologique a été complété par une échocardiographie transœsophagienne (ETO), et une IRM. Le traitement chirurgical a consisté en la fermeture chirurgicale du diverticule avec annuloplastie mitrale.

Cardiac diverticulum is an infrequent congenital malformation, it's even more rare in adulthood. It's often associated with other thoraco-abdominal diverticulums, and rarely isolated. The diagnosis relies on echocardiography. By the way, the magnetic resonance imagery (MRI) allows a finer analysis of the diverticulum, its topography and its situation contributed to vascular and cardiac structures, it has largely replaced the ventriculography. We report in this article the case of a patient of 36 years whose diagnosis of the diverticulum was made by transthoracic echocardiography (TTE) at the time of an assessment of dyspnea, this exam also objectified a mitral insufficiency by deformation and dilatation of the mitral annulus. Radiological assessment was completed by a transoesophageal echocardiography (TOE) and magnetic resonance imagery (MRI). Surgical treatment consisted of surgical closing of the diverticulum and mitral annuloplasty.
