RésuméLes sténoses serrées du tronc commun (TC) sont souvent très symptomatiques, et constituent une contre-indication traditionnelle aux tests d’ischémie ; lorsque ceux-ci sont réalisés, une ischémie étendue est induite fréquemment associée à un riche cortège symptomatique. Nous rapportons l’observation d’un patient de 69 ans complètement asymptomatique chez lequel à la fois une épreuve d’effort une échocardiographie à la dobutamine maximales ont été faussement rassurantes ayant écarté l’éventualité d’une ischémie significative en dépit de la présence d’une sténose critique du TC ultérieurement confirmée par la coronarographie. Le 2D strain sous dobutamine était rétrospectivement pathologique et aurait pu redresser le diagnostic.
Left main coronary artery severe stenosis is frequently very symptomatic and is considered as a classical contraindication for non-invasive ischemia testing. When performed, those tests usually yield a large area of ischemia associated with a rich symptomatology. We report the case of a 69-year-old completely asymptomatic man in whom both treadmill effort testing and dobutamine stress echocardiography were falsely reassuring as they eliminated significant ischemia despite the presence of a critical stenosis of distal left main artery ultimately confirmed by angiography. Dobutamine 2D strain performed afterwards was pathological and could have corrected the initially reassuring diagnosis.
