Endocardite tricuspide du post-partum à streptocoque B

RésuméLes endocardites à streptocoque B sont très rares. Elles intéressent essentiellement les valves du cœur gauche, l’atteinte de la valve tricuspide par ce germe reste exceptionnelle. Nous rapportons l’observation d’une jeune patiente de 36 ans ayant présenté en post-partum une endocardite tricuspide à streptocoque B. Elle a été traitée par céphalosporine de troisième génération et aminoside. L’évolution au bout de cinq jours d’antibiothérapie a été marquée par une destruction valvulaire rapide avec aggravation de l’insuffisance tricuspidienne aboutissant au décès de la patiente. Il s’agit du 23e cas d’endocardite tricuspide à streptocoque B rapporté dans la littérature. La mortalité de cette pathologie atteint 36 % en l’absence de traitement chirurgical. L’approche médicochirurgicale est donc le traitement de choix de ces patients.

The B streptococcal endocarditis are very rare. They primarily affect the left heart valves, the achievement of the tricuspid valve is exceptional. We report a young patient aged of 36 years who presented in postpartum a tricuspid endocarditis with streptococcus B. She was treated by third-generation cephalosporin and aminoglycoside. The evolution after 5 days of antibiotic therapy was marked by a rapid valvular destruction with worsening tricuspid insufficiency leading to death of the patient. This is the 23rd case of tricuspid endocarditis streptococcal B reported in the literature. The mortality of this disease reached 36% in the absence of surgical treatment. The medico-surgical approach is the treatment of choice for these patients.
