Prise en charge du syndrome coronaire aigu à la phase aiguë

RésuméLe bénéfice clinique de l'association de l'aspirine et du clopidogrel pour prévenir la survenue de nouveaux accidents coronaires après un syndrome coronaire aigu reste l'acquis majeur de ces dernières années dans la prise en charge de la maladie coronaire. Ceci est renforcé par le fait que ce bénéfice s'étend également à la prévention des complications thrombotiques de l'angioplastie avec stent avec des modalités d'utilisation strictement identiques de cette bithérapie anti-agrégante. La concordance des résultats des différentes études cliniques sur l'intérêt de la bithérapie anti-agrégante orale par rapport à la monothérapie par aspirine est essentielle pour le clinicien. Les résultats des essais cliniques génèrent toujurs de nouvelles hypothèses et de nouvelles questions pratiques non encore résolues. La dose de charge en clopidogrel et l'intérêt d'une surveillance biologique de la réponse au clopidogrel pour en améliorer le bénéfice en sont un exemple typique. Ces questions constituent un véritable enjeu clinique pour lequel une réponse adéquate permettrait une utilisation plus rationnelle des antiagrégants plaquettaires oraux actuellement disponibles. Nous discutons ici l'ensemble de ces points.

The clinical benefit of the combination of aspirin plus clopidogrel over aspirin alone to prevent recurrent events after acute coronary syndrome is obviously a key step of the past few years in the management of coronary artery disease. The extended benefit of this combination among patients undergoing stent implantation is another key message for clinician. The consistent benefit of the dual oral antiplatelet therapy in all clinical trials is reassuring for clinicians. However, all well designed clinical trials generate new hypothesis and unsolved clinical issues. The loading dose of clopidogrel and whether we should monitor the biological response to clopidogrel in order to improve its clinical benefit are the next clinical challenges for clinicians dealing with acute coronary syndromes. Providing adequate answers to these relevant clinical issues should improve the clinical benefit of clopidogrel. All these important points are discussed in the following manuscript.
