•Isolated cerebral mucormycosis is a rare fungal infection of the nervous system.•Risk factors include IV drug abuse, poorly controlled diabetes, and immunocompromised status.•Despite prompt diagnosis and treatment, morbidity, and mortality remain high.•Diagnosis and treatment guidelines for isolated cerebral mucormycosis are presented.
