•Study population consisted of uniform group of patients with MTLE.•Majority (87%) had pathologically verified hippocampal sclerosis.•Selection for surgery was not influenced by Wada results.•One of the largest study to report the seizure outcome in patients with Wada failure.•Wada test was found to be only modestly useful for predicting seizure outcome.
