Abstract
Multiple systemic atrophy is a neurodegenerative disease affecting the extrapyramidal, cerebellar, autonomic and pyramidal pathways. Among the classical symptoms, nocturnal stridor, which consists in a noisy breathing due to a turbulent airway in the larynx, has been associated with poor prognosis. Its pathology is still not well understood and both dystonic and neurogenic features have been found and held responsible for this symptom. Treatments for the stridor have been shown to reduce mortality. Several more or less invasive therapies, including continuous positive airway pressure, botulinum toxin and tracheostomy or laryngeal surgery, can be discussed.
