This case report describes the treatment of an adult patient of 26 years and 9 months old bimaxillary protrusion and a 12 mm space in the area of the maxillary left first molar tooth, the left and right mandibular first premolars and maxillary first premolar were extracted to retract the maxillary and mandibular anterior teeth lingually as much as possible while reinforcing the anchorage of the molar region by headgear, transpalatal arch, and Class III elastics for anchorage preparation. We decided to utilize the space of the maxillary left first molar region. Normal overbite and overjet were achieved and an excellent function was obtained. The clockwise mandibular rotation did not occur. It was possible to retract the maxillary and mandibular anterior teeth. As a result, the facial profile improved substantially. Two years after retention, his occlusion remained stable.
