Abstract
Transsphenoidal access to the petrous apex is a good alternative to lateral approaches. This acces is of choice when the cholesterol granuloma reaches the postero-lateral wall of the sphenoid sinus. The transsphenoid endoscopic approach, unlike lateral approaches to the petrous apex, does not disturb cochlear and vestibular function and allows postoperative endoscopic follow-up.
