Abstract
Ischemic cerebrovascular disease (transient ischemic attack and stroke) and diabetes mellitus are highly prevalent entities in our society. They are closely interlinked, being diabetes at same time a risk and prognostic factor for stroke. The prevention of stroke in patients with diabetes require a multidisciplinary approach, with interventions aimed at reducing metabolic stress, high blood pressure control and diminishing levels of LDL-cholesterol. The use of certain antidiabetic drugs (thiazolidinediones and sulphonylureas) may be relevant in acute cerebral ischemia. However the drug of first choice will be metformin.
