Abstract
Epidemiological data are highly consistent: the risk of microvascular complications of diabetes is positively and closely linked to glucose control observed in the years preceding the occurrence of complications. Moreover, these complications, especially nephropathy, prepare the forthcoming macrovascular complications: no nephropathy, no excess of cardiovascular diseases. But the microvascular risk is modifiable: in type 1 diabetes, but also in type 2 diabetes, intervention studies are conclusive. A good glucose control minimizes sustainably the risk of microvascular complications. When exposure to hyperglycemia was longer, the benefit of the ambitious glycemic control becomes tangible aftera delay depending on the duration of diabetes, a parameter to be considered for individual assessment of targets.
