Abstract
Type 2 diabetes (T2DM) is due to both insulin deficiency and insulin resistance. In cases of obesity or some rare monogenic syndromes with severe insulin resistance, intensification of insulin treatment may be totally ineffective. In these specific situations, the use of concentrated U-500 regular insulin (Umulin-R® U-500) in a portable pump may constitute an interesting alternative for improving both insulin absorption and metabolic control. We report herein our preliminary results in 12 T2DM patients with severe insulin resistance treated with U-500 regular insulin, with an improvement of HbA 1c after 3 months without body weight gain and no increase of hypoglycemic events.
