Abstract
The beneficial effects of bariatric surgery on type 2 diabetes mellitus (T2DM) have been extensively documented. However, the mechanisms underlying the amelioration of glucose metabolism after this type of surgery are not fully understood. Recent studies suggest that -cell function is a better determinant of the long-term remission of T2DM rather than the evolution of insulin sensitivity or GLP-1 secretion. Most importantly, bariatric surgery can restore the altered -cell acute insulin response to glucose as observed in T2DM suggesting that this important defect is reversible. However, these important beneficial effects published in diabetic patients with BMI >40 kg/m2 have to be confirmed in patients with BMI from 35 to 40 kg/m2.
