Abstract
Bariatric surgery has emerged as a highly effective treatment not only for obesity but also for type 2 diabetes. Meta-analysis reported complete resolution of type 2 diabetes (T2D) in 78.1% of cases after a bariatric surgery in morbid obese patients. The extraordinary results obtained in diabetic patients with BMI >35 kg/m2 have led investigators to query if similar results could be achieved in patients with BMI <35 kg/m2. Some preliminary studies suggested that metabolic surgery is safe and effective in T2D patients with BMI <35 kg/m2. However, it has been also demonstrated in other studies that metabolic surgery is less efficient to promote T2D remission in patients with BMI <35 kg/m2. These controversies about the metabolic surgery demonstrate the need of additional long term studies in this specific population in order to better define the safety, effectiveness, and cost effectiveness of metabolic surgery in patients with T2D. In 2015, it is premature to state that metabolic surgery is an accepted treatment option for T2D with BMI <35 kg/m2.
