Abstract
Initially patient education was developed in the setting of hospital organizations. However to respond to the challenge of the increasing number of patients with chronic diseases and the subsequent need of implementing outreach community educational programs, new modes of organization have to be proposed. Multidisciplinary management programs (MPP) have been developed in the early 2000's trying to respond to this question. Hence since 2004, the Nantes's RespecticÅur Heart Failure Network has developed a multidisciplinary management organization based on structured patient education programs and coordination of care, aiming to offer to each patient a local personal care plan. Based on a flexible, tailored and accessible organization, this program and others have demonstrated their beneficial effects on quality of life, reduction of hospital readmission and survival. In the context of new directives from the French National Health Agency (HAS) aiming to further implement education intervention in the setting of ambulatory care, we discuss in this article the roles which can be endorsed by the MPP teams. These teams have developed highly specific educational and organizational skills and we strongly suggest that they should be asked to help primary health care professionals to implement education programs in the setting of multidisciplinary primary care centers.
