Use of fenofibrate is worth considering in uncontrolled major HTG despite strict dietary treatment. Statins remain the established treatment in cardiovascular prevention but they have never been evaluated in major HTG. The association statin-fenofibrate is possible in secondary prevention when triglyceridemia remains uncontrolled with low HDL despite statins titrated at optimal doses in high risk patients.
