Abstract
Most patients with type 1 diabetes should be offered an intensive insulin regimen (Basal/Bolus) which, when combined with self care management education (SME), is the best way to achieve glycemic targets. The Continuous Subcutaneous Insulin Infusion (CSII) is the gold standard for the intensification of glycemic control. The prescription is regulated in order to assure both metabolic efficiency and security. CSII is a tool which is at the patient's disposal. Intensive Blood Glucose Monitoring (BGM) guides the daily adjustment of insulin dose. The patient with a pump should be able to switch to multiinjections regimen in case of technical failure in order to avoid ketoacidosis. A skillful application of the CSII needs regular and reinforced SME. At the hospital “Hôtel-Dieu de Paris” we organized an ambulatory day care structure based on group education, in order to promote and strengthen the skills in self care management, technology mastery and security requirements.
