Abstract
Defects in comprehension and insufficient knowledge from patients affected with chronic diseases are major causes for health acute events. Favouring a joint patients-health caregivers approach seems more efficient to assist them in solving these difficulties. A chronic disease leads to more complex intervention strategy, and beyond clinical and biological data, imposes investigations in psychoaffective, socioprofessional, educational and motivational fields. As a basis for any Therapeutic Education, the educational diagnosis is based on the patient's expression of his/her disease experience, the understanding of patient's attitudes towards health, the collection of useful information, in view to propose an educational and accompaniment process making sense for the patient. It can't be definitive, nor exhaustive. In phase with patient's evolution, such practice favours understanding, engagement and innovative alternatives. Its modalities are conceived to inform the patient on the evaluation intents, to induce his/her active participation, to assist him/her in an autodetermination, with practical applications perspectives, efficient in his/her daily life. It is part of an educational strategy integrated within health taking into account the disease' complexity and the own patient's resources, under management and responsibilities of healthcare professionals. Leading to profound changes in healthcare methods and organization, it imposes to health professionals the acquisition of a dual care and educational competence as well as multidisciplinary and cooperative practices.
