Adrenalectomia posterior retroperitoneoscópica – introdução da técnica num hospital generalista

ResumoA adrenalectomia posterior retroperitoneoscópica, técnica cirúrgica relativamente pouco utilizada na abordagem da glândula suprarrenal, parece apresentar algumas vantagens sobre a via transperitoneal, nomeadamente uma menor necessidade de analgesia, menor tempo operatório e menor tempo de recuperação globalO principal óbice à adesão a esta via tem sido o escasso «domínio» que os cirurgiões gerais e endócrinos têm do espaço retroperitonealEste trabalho tem como objetivo primordial a divulgação da técnica, descrevendo as dificuldades encontradas na sua implementação num hospital geral e analisando os primeiros 11 casos operados.Com base nesta experiência inicial, identificam‐se as condições consideradas indispensáveis para a execução segura da técnica.

Posterior retroperitoneoscopic adrenalectomy, a seldom performed surgical approach to the adrenal glands seems to offer several advantages when compared with the transperitoneal approach. They include, among others, lesser need for analgesia, inferior surgical time and faster global recuperation period.Its main drawback has been the fact that most general and endocrine surgeons are unfamiliar and feel unconfortable when dealing with the retroperitoneal space area.It is our main purpose with this paper to contribute to a better understanding and knowledge of this surgical procedure. The steps that led to its implementation among us are summarily described. Our first eleven patients are reported.This approach is discussed at the light of our experience and the verified outcomes, particularly in what concerns the technical and logistic conditions that are deemed mandatory to allow it to be properly and safely performed.
