Abstract
Since 1990, the community geriatric unit has been providing coordinated care for dependent and frail mostly homebound older adults, presenting complex medico-psycho-social issues in partnership with community services. Its components are a multidisciplinary mobile team that provides comprehensive geriatric assessment at the patient's home with case management and long-term follow-up including home palliative care, as well as a 24Â h/7 day a week medical call service. A day hospital with therapeutic activities is also part of its services.
