Abstract
The PAERPA national program aims to implement flexible and coordinated healthcare itineraries by reinforcing the integration of local professional actors. The elderly can be characterized, among other things, by the presence of criteria of frailty. The identification of frailty, which is central to this project, raises a number of challenges. One of these challenges is the definition of the target population concerned by the project. The mobilization of the different actors can be made more or less easy or effective by this definition. Care must be taken to respect the freedom of individuals to choose the professionals to be involved and medical confidentiality needs to be complied with when disseminating information concerning the elderly people receiving care under CCP (coordination clinique de proximité [coordination at local level]). CTA (coordination territoriale d'Appui [territorial back-up]) will facilitate the integration of local players. An adjustment to the legislative framework was made in December 2012 to facilitate the dissemination of information among all the actors in geriatric care.
