Abstract
Bibliographic research has helped us to build a consistent and reliable protocol. It provides nurses with sufficient autonomy to reduce the charge in activity while maintaining satisfactory medical care. With the arrival of new oral anticoagulants (NACO), the drafting of a management protocol INR might appear inappropriate. However, a protocol is justified by the frequency of VKA treatments in nursing homes and the difficulties encountered in the organization of therapeutic monitoring. Another argument, the cost of new therapies is inconsistent, given the frequency of the CAAF in the elderly, with budgets for medical and social structures. This work at the crossroads of medical, economics, and organization is essential to ensure geriatric care of quality.
