Abstract
Fahr's syndrome (FS), defined as bilateral symmetrical basal ganglia calcifications, preferentially occurs in patients with dysparathyroidia. This rare disorder is characterized by its clinical polymorphism but includes in the first line neuropsychiatric manifestations. We report a case of FS with atypical clinical revelation in a 71 years old patient, with left side heaviness and slowing down motion. The clinical examination objectified a Parkinsonian syndrome with apathy. The CT scan showed bilateral and symmetrical calcifications of basal ganglia and cerebellar dentate nuclei. The biological assessment leads to pseudo-hypoparathyroidism. Substitutive therapy with calci-vitamin D drugs associated with symptomatic treatment (dopatherapy) and motor rehabilitation has allowed a moderate improvement in apathy and motor symptoms.
