Abstract
In the elderly, two types of cognitive impairment may occur after surgery: confusional syndrome and postoperative cognitive dysfunction. Confusional syndrome is a serious problem associated with significant morbidity and considerable mortality. Management is based on screening, identification of the underlying causes, on supplementary measures and appropriate pharmacological treatment. Postoperative cognitive dysfunction is described late after surgery. Because of the multifactorial nature of the disorder, primary prevention based on a neuropsychological assessment of risk factors performed by a specialist is required. Recent studies have raised more questions than answers.
