Abstract
At the Intercommunal Hospital Complex of Creteil, the mobile geriatrics team works every morning in the acute care unit in order to contribute to the care and orientation of the patients aged over 75Â years. In the unit, the nurse works in partnership with the geriatrician, providing a comprehensive assessment of the patient in collaboration with the unit's medical team. The mobile team nurse then recommends appropriate nursing care depending on the needs of the elderly patient, focusing on the prevention of risks of loss of autonomy. However, in an acute care unit, an overall geriatric assessment faces certain limitations due to the specificity of acute care. Although the participation of a mobile geriatric team within the framework of hospital care involves specific difficulties, the team at the Intercommunal Hospital Complex of Creteil has obtained positive results, providing suggestions for improvement of the care of old patients requiring hospitalization.
