Polysensibilisation rare aux médicaments impliquant plusieurs mécanismes d’hypersensibilité

RésuméNous décrivons le cas d’une patiente présentant trois types d’hypersensibilité aux médicaments : hypersensibilité allergique immédiate (HSIA) à la pristinamycine, hypersensibilité allergique retardée à la clindamycine, et hypersensibilité non allergique (HSNA) aux bêtalactamines. Les diagnostics ont été portés sur l’histoire clinique très évocatrice, et des tests allergologiques cutanés et biologiques pertinents. Il s’agit du premier cas décrit associant trois mécanismes d’hypersensibilité. Les mécanismes physiopathologiques sous-jacents restent imprécis. Chez la patiente, les diagnostics pouvaient être évoqués dès l’anamnèse, ce qui souligne le rôle de l’interrogatoire dans l’orientation de la nature des tests allergologiques.

We describe the case of a patient with three types of drug hypersensitivity: immediate allergic hypersensitivity to pristinamycin, delayed allergic hypersensitivity to clindamycin, and non-allergic hypersensitivity to betalactams. Diagnoses were made on the very suggestive clinical history, and relevant skin and biological tests. This is the first reported case involving three mechanisms of hypersensitivity. The pathophysiological mechanisms remain unclear. In the patient, the diagnosis could be suspected from the history, which emphasizes the role of anamnesis in the choice of the kind of allergy testing.
